Bad, you're absolutely right. Harlan Ellison put it this way: if you write a story in which nobody can use his left hand, under any circumstances, and abide by it throughout, then you will indeed have shown internal logic. But if your hero is desperate, and there's seemingly no way out, and you let him use his left hand to get himself out of a jam, the reader is going to feel lied to, conned and duped. And he's going to be angry. Justly so. (The above isn't exact by any means, but it delivers the idea.)
I think most movie makers today are just lazy and careless. Check out the IMDb "Goofs" selections to see just how much is overlooked by them- even during editing.
