Bonnie - Did not mean to imply that there is not a learning curve for drugs post approval. That I believe falls into the third bioethical category I listed. Many drugs I know are utilised for conditions for which they are not indicated based on original research, but seem likely to be of benefit inductively. An example may include the increasing use of IVIG for connective tissue diseases ( Rheumatoid arthritis, Kawasaki's disease etc )
An interesting parallel occurs in Medical education where physicians in training (interns, residents ) are supervised for a fixed period of time and upon certification are allowed to practice unsupervised with the expectation that they will continue to learn but in an auto-didactic way.
