The main character is alive (though not well) at the end of either version. The last scene (UK version) actually ties the whole movie together quite nicely and makes you question a lot of what came before. I also had some trouble keeping the characters straight on the first viewing, but it was such a compelling film that I re-watched it a couple of times more.
It's a very well made film with genuine scares and a thought-provoking ending (plus Juno is quite the hottie). This is a scary film that I could watch over and over. I give it one bomb for sheer enjoyment factor.
