He APPARENTLY jumped off the roof of an eight story building. However, popular speculation has it that he was actually killed by the Yakuza. The circumstances off his death are very suspicious - he had little reason to kill himself, amongst other inconsistencies. The Yakuza were pissed off at the way Itami portrayed them as bungling idiots. Following the release of "Minbo no onna" in 1992, Itami was attacked by a group of Yakuza who severely slashed his face. He is said to have worn the scars as a badge of honor.
