"Misery" left a lot out that wasn't in the book, like when she cuts off his toe (it's been a long time...I know there was some act of dismemberment that wasn't in the film) or put a nail in his hand (yet again, I forgot, but I remember seeing the movie, reading the book, and then feeling a vast emptiness inside).
"Stand by Me", also by Reiner, was pretty faithful and "Shawshank Redemption" was, too, but "Carrie" included every last detail and didn't include anything. The only closer to exact reproduction it could come is if they just flashed news clippings and journal entries over the screen, what the book was made up of.
