You nailed the Big Three: Boston, LA, and DC. For entirely separate reasons, I've found these cities to have the worst driving conditions in America.
LA: too many lanes, too much traffic, maniac drivers
Boston: too few lanes, poorly marked and confusing, lousy drivers
DC: nonsensical traffic patterns that trap you, and the most poorly designed piece of infrastructure in America: The Beltway.
