Yes, most of them go through "sex-reassignment." (Very few of them actually feel comfortable with the gender they were assigned to, though. Not because they feel they are "the other gender," but because they feel they are NO gender, or perhaps a third gender.)
The problem is that, even after surgery, some of them *still* don't have a clear sex. Someone can have a penis with female chromosones. That is, when we say someone is male or female "inside," we're not always talking about psychologically. Sex and gender are not only matters of external genitalia. And doctors can't change our chromosones.
Of course, I realize I'm preaching to the choir here, in many respects.
