... and not the one Crowe had in mind.

The voice at the end is not that of Sofia. It's a nurse in the future waking him from his lucid dream. (As Crowe says in the DVD commentary).

In "Real life", David Aames never saw Sofia again after the scene lying on the road. He never got his face fixed. He signed up for LE and spliced his lucid dream with his real life from that point. His feelings of guilt caused his subconcious to to dream that he murdered Sofia. And he chose to wake up 150 years in the future when they could cure his suicide. Tech Support had the real story.

The question everyone asks is why didn't Aames splice the dream into his life before the crash. Crowe answers this by saying that Aames wanted to feel pain to enjoy his dream all the more. (The whole sweet and sour thing) because he had led a charmed life up to that point.
